Abstract

Midwifery care includes maternal and child health, covering pregnancy, childbirth, postpartum, neonates, and family planning. This case study aims to provide midwifery care through continuity of care using the midwifery management approach. The case report involves a third trimester pregnant woman, delivery, postpartum, neonatal, and family planning care at the private midwifery practice of Siti Khasanah, using step SOAP method, which includes Subjective, Objective, Assessment, and Planning. Midwifery care was provided to Mrs. H, G2P0A1, 22 years old, starting from 38 weeks of gestation, through delivery, postpartum, newborn care, and family planning, from Februari 24, 2025, to June 16, 2025. The mother was categorized as high-risk pregnancy (HRP). The second stage of labor lasted 45 minutes, spontaneous delivery at term, with strong crying, active movements, assisted by a midwife, male baby, birth weight: 3270 grams, length: 52 cm, early initiation of breastfeeding (EIBF) for 1 hour. The placenta was delivered spontaneously, and the postpartum period was normal. The mother chose the 3 month injectable contraceptive. The case report results showed that midwifery care from pregnancy to family planning had no complications. The mother was able to care for herself and her baby. Her knowledge improved, starting with maintaining her health during pregnancy by ensuring adequate rest, as she experienced sleep difficulties in the third trimester. She also learned how to increase breast milk production and choose the most suitable contraceptive method. Thus, the problems she experienced were resolved without complications. After continuous care and intervention, it can be concluded that Mrs. H’s pregnancy was high-risk with a KSPR score of 10 (yellow category). The care for pregnancy, childbirth, postpartum, newborn, and 8 family planning proceeded normally. It is expected that after receiving the care, Mrs. H will be able to independently apply the care to herself and her baby.
